HeartFormed Privacy Policy

HeartFormed is a Christian ministry that offers resources for heart change, including the HeartFormed Compass assessment, the HeartFormed Map, the HeartFormed 4T tool, and related emails. This page explains what information we collect and how we use it. If you have questions, write to jeff@heartformed.com.
What we collect
Your first name, last name, and email address, which you give us when you take the Compass assessment or use another HeartFormed tool.
Your answers to the assessment questions and the results they produce.
Basic website information, such as which pages are visited, collected by our website provider.
How we use it
To show you your results and send your personalized HeartFormed Map.
To send you a short series of emails after the assessment, and occasional emails about HeartFormed resources.
To understand how people use our tools and improve them.
We do not sell your information, and we do not share it with anyone except the services that help us run HeartFormed.
Services we use
Your information is handled by Squarespace (our website), ScoreApp (the Compass assessment), Kit (our email service), and Fillout (the 4T tool). Each of these has its own privacy policy. Your information may be stored on servers in the United States.
Your choices
Every email we send has an unsubscribe link at the bottom. Click it any time and you will stop receiving emails.
To see, correct, or delete the information we hold about you, email jeff@heartformed.com and we will take care of it.
What these tools are, and are not
The Compass, the Map, the 4T, and our emails are for spiritual growth and education. They are not counseling, therapy, diagnosis, or treatment, and using them does not create a counselor-client relationship. If you are in crisis or thinking about harming yourself, please call or text 988 (in the US) or contact local emergency services right away.
Children
HeartFormed tools are not meant for children under 13, and we do not knowingly collect their information.
Changes
If we change this policy, we will update the date at the top of this page.
